IEP Falsification Survey

At MPACT, we receive many reports of IEP Alteration, Falsification and Forgery.  We are working to identify, validate and address concerns such as these on a system-wide basis.  Please help us to improve the educational outcomes for our children and special education services by completing this survey now.

This picture was taken in one of our school district's elementary schools.  The box is used as a disciplinary measure for children with disabilities.  This is only used for children with disabilities.

Does your school district have a similar box (or perhaps small closet) that children are placed in?  If you know of this type of behavior management "space" used at your child's school, please let us know.  Please include the dimensions of the box, if the door has a lock, if there is a window, if there is light.  Please take a photo of it.  We may ask you to send us a copy.
